OBJECTIVES FOR OBTAINING GLOBAL SUBSIDY.

This project aims to:

(a) Create prevention campaigns of breast cancer.

(b) Acquire a digital mammography unit, which can perform 50 mammograms a day with stereotactic procedures, equipping the mastology and radiology service of Oswaldo Cruz University Hospital of Pernambuco, and offering free preventive examination and treatment.

(c) Serve as a reference for academic research and university studies.

BENEFICIARIES OF THIS GLOBAL SUBSIDY.

This project aims to benefit mainly poor patients of the Oswaldo Cruz University Hospital of Pernambuco (HUOC), which are users of SUS - Sistema Único de Saúde is the public health system in Brazil which aims to help poor people in need of medical assistance for free - in the state of Pernambuco, and who would be able to obtain high-quality examination and follow-up in the field of mastology. It will also benefit professionals and students in the medical field.

The HUOC has only one analog mammography unit without stereotaxy which has been used for 15 years. It is a device that can perform only pre-surgical markings and biopsy of non-palpable suspicious lesions that do not have sonographic expression. In most of these cases, in which the patients' situation require further research, they resort to the private health system, having to pay for this procedure that is extremely important for diagnosis and proper treatment.

A new mammography unit with stereotaxy would be able to perform up to 50 tests a day. I.e., in addition to double patient care, which would result in an amount of about 1,000 mammograms a month. Not to mention all stereotactic procedures that are essential to the diagnosis of breast cancer (which HUOC does not offer nowadays).

PERIOD FOR PROJECT'S ACHIEVEMENT.

The project has been carried out since October 2015, through campaigns to raise awareness, prevent and combat breast cancer.

SUSTENTAINABILITY. COMMUNITY NEEDS THAT WILL BE MET BY THE PROJECT.

Pernambuco is the state that has the highest incidence rate of breast cancer in the Northeast of Brazil, with 2,400 new cases per year, while Brazil has a rate of 57,000 new cases. The fact that there are still thousands of women who do not have access to resources and preventive examinations as mammography, for instance, is alarming. Mammography is the only test that is preventive and reduces mortality from this disease when early diagnosed, reducing expensive mutilating surgeries, reconstruction, prosthetics, radiotherapy and chemotherapy. It is not a coincidence that breast cancer is the leading cause of women death in Brazil (15 thousand deaths per year).

In 2015, in the State of Pernambuco, 189,225 mammographies were carried out and SUS itself performed 117 pre-surgical markings on non-palpable lesions through stereotactic procedures. This is not enough to attend the great number of women aged between 50 - 60 years old (622,733) and is far less to attend those aged between 40 - 65 years old (1,219,000). Throughout the survey, it was noticed that only one philanthropic institution that performs mammography with stereotaxy is able to meet local needs in all State of Pernambuco. No state public hospital is equipped to offer the use of stereotactic procedures, which would allow the realization of directed biopsy with image.

SUSTAINABILITY. COMMUNITY INVOLVEMENT IN BOTH HEALTH AND ACADEMIC AREAS.

The Oswaldo Cruz University Hospital of Pernambuco (HUOC) was chosen because of its being public, supportive to the university and highly qualified technically. It is the clinical hospital of Pernambuco University (UPE) and has financial resources provided by contracts with SUS - intermediated by the State Health Department of Pernambuco, which is the state manager of SUS. Also, it is an economically stable institution and has a historic continuous improvement in its activities. The HUOC is used by UPE undergraduate students in order to get practice (training) in Medicine, Nursing and Biological Sciences, as well as by residents in the areas of Medicine, Nursing, Nutrition and Palliative Cares. The HUOC has a qualified human team able to double its service in the area of mastology, depending only on a new mammography unit to achieve this goal.

SUSTAINABILITY. TRAINING, COMMUNITY CAMPAIGNS AND EDUCATIONAL PROGRAMS.

The HUOC already has highly qualified healthcare professionals to handle a digital mammogram equipment as well as to perform the stereotactic service. However, it does not have such equipment yet. In order to maintain a high quality service, the HUOC has committed to carry out periodic training for the mastology team, as well as for other professionals directly related to the use of the mammogram equipment.

FOCUS AREA. GOALS THE ACTIVITY WILL SUPPORT AND THE WAY TO ACHIEVE THE GOALS.

The project serves the area of Prevention and Treatment of Diseases. Thus, the following criteria will be met: (a) Promoting the training of health workers; (b) Promoting disease prevention programs with the aim of reducing the incidence of complications from non-communicable diseases; (c) Improving the health infrastructure of local communities.

SUSTAINABILITY. SELECTION OF THE COLLABORATING ORGANIZATION AND ITS CONTRIBUTION.

(1) The Oswaldo Cruz University Hospital of Pernambuco was chosen to celebrate an institutional partnership with for its offering the proper place, operating and maintaining the mammography unit by qualified professionals and contributing to studies and academic research, by serving poor people for free as well as directing people diagnosed for proper health monitoring, recording statistical data, allowing the monitoring of Rotary and contributing to the improvement of medical knowledge to students, researchers and health professionals.

The HUOC mobilizes units and teams in the areas of medicine, nursing, pharmacy, dentistry, nutrition, social assistance, psychology, physiotherapy and occupational therapy.

The mastology service is one of the most well-known in the state of Pernambuco. Since 1978, mastology has been a school subject performed by Pernambuco University of Medical Sciences - the service is a benchmark in the Northeast. It also counts on a qualified team of professionals from various subjects which attend approximately 50 patients a day at the clinic.

(2) The Warrior Women Calendar, a group formed by women who fight or have already fought against cancer and who are in contact with so many other patients and associations. The Warrior Women Calendar project, which is already in its fourth edition, was designed by the delegate Verônica Azevedo, who faced breast cancer in 2012. In 2014, she sought the help of Rotary in one of its campaigns and in 2015, the Rotary Club of Recife Casa embraced the struggle of these warriors by identifying a need in the fight against the disease, which motivated them to idealize this project.

BUDGET. DESCRIPTION OF THE MAMOGRAPH SELECTION PROCESS.

Based on the technical information provided by professionals of mastology, the price of the mammogram equipment was estimated by three offers provided by companies suppliers of mammography units. The purchase of a lower-priced equipment, quoted by GE Healthcare, was approved on January 5th, 2016. The selected mammogram equipment is adequate to local cultural and technological standards.

SUSTAINABILITY. PROPERTY AND MAINTENANCE OF THE MAMMOGRAPHY UNIT.

The mammography unit acquired by this project will be donated to the partner institution, which will bear the responsibility to preserve, maintain and use the equipment in the best way, following the objectives outlined in this project and enabling Rotary to make periodic inspections and receive productivity reports.

FUNDS AND SUSTAINABILITY. FINANCIAL RESOURCES TO ENSURE THAT THE RESULTS OF THE PROJECT WILL BE KEPT ON THE LONG TERM.

According to an agreement established between the HUOC and SUS, every medical examination will be financed by the later, not depending on financial support from Rotary or any patient.

DISCLOSURE. DEEDS ALREADY PERFORMED IN 2015, 2016, 2017 AND 2018.

(a) Institutions: Using media of public institutions to spread the importance of the project, aiming to reach professionals and citizens who work for the following departments: Court of Justice, Prosecutors, Civil and Military Police, Fire Department, Municipal and State Health Departments;

(b)Stadiums and Soccer Clubs: With the support of FPF (Soccer Federation of the State of Pernambuco), soccer clubs like Náutico, Santa Cruz and Sport, used soccer games to promote this campaign and expand the awareness of the fight against breast cancer;

(c) Hospitals, Clinics and Treatment Centers: Exposing the campaign with printed materials (pamphlets, posters and banners);

(d) Press: Using the media space for social responsibility, such as television, radio, newspapers, internet, and Municipal and State Health Departments;

(e) Social networks: Posting photos and texts on the subject and the deeds developed by the project, seeking to raise awareness and collaboration. A 'fanpage' (www.facebook.com/mamografodorotary) was created for that matter;

(f) Lectures: Conducting lectures at strategic locations with the participation of trained professionals or treated patients and at Rotary club meetings;

(g) Exhibitions: sell calendars and gifts at aeroport and shopping centers in four cities (Recife, Vitória de Santo Antão, Bezerros, Serra Talhada...);

(h) Bike Ride: Pedala UPE 2017
