Summary: This project will provide approximately 250 hours of respite care/home health/well check/nursing services to families working with the dx of Alzheimer's/Dementia
Goshen County continues to serve an aging population with an increase in the diagnosis of Alzheimer's and Dementia. Our community has come together over the past 5 years and identified different resources to support a successful respite program for families dealing with this diagnosis. The need continues on, so our need for bringing more resources to this project also continues. This project will meet the needs of approximately 250 hours of this desperately needed skilled care. The average hourly rate is around $30 per hour. Over the next 18 months, club members will continue to seek resources for this project, while allowing this funding request to serve current needs. Members will be directly involved in public service announcements, PR designs through flyers, brochures, and social media announcements, referrals, and working with the skilled staff of the respite program to make sure all needs are being met for these patients.

Brett,
Torrington has a grant P-2504 that is over two years old without an update. If the grant is complete a completed & signed District final report and a copy of the receipts, checks or bank statement equaling the total of the grant needs to be uploaded to the documents area. If it is not complete a history log update as to the status of the project needs to be completed. Until one of these is completed the club does not qualify for this grant.
Also, please upload a detailed budget of the total income and expenses for this grant to the documents area.
